I do not know how obesity affects the body composition of the construction of the body / (which makes you look bigger but in a bad way, I think?), but obesity makes it more difficult for the heart to pump blood to all parts of your body since it has to pump blood a greater amount of body mass. As the heart pumps blood to the lungs to get oxygen into the blood, lungs also work (the attempt to get oxygen in the blood). Any extra weight (more than dead weight, which differs from one person to person, depending on how big business as usual bones, activity, etc.) puts additional pressure on the heart and lungs to get oxygenated blood throughout the body.
